Peter Christensen Health Center provides a comprehensive care designed to help you manage your health in the most convenient way possible.
Care is provided to the Lac Du Flambeau Community, Vilas, Oneida, and Iron counties. Our industry-recognized approach to coordinated care guides our focus in clinical excellence.
We offer group and 1:1 education sessions for persons who have been diagnosed with diabetes, or are at an increased risk for developing diabetes, and their families.

We are responsible for developing, executing and administering a comprehensive environmental health program designed to improve the health status of the Lac du Flambeau Tribal community.
We would like to help you get the most of your first visit with your primary care provider (PCP). Your time with your PCP is valuable and we have a few suggestions to help you make the most of your visit. To begin, we ask you to complete the New Patient Information packet that is included with this welcome kit. Please make sure you:
Complete the New Patient Registration Form.
Complete the Health History Questionnaire.
Review and sign the Notice of Privacy Practices.
Complete the Authorization of Release to furnish information and Assignment of benefits (Private Insurance, Medicare, and Medicaid).
Review the Contract Health Booklet.
