Patient Resources

We would like to help you get the most of your first visit with your primary care provider (PCP). Your time with your PCP is valuable and we have a few suggestions to help you make the most of your visit. To begin, we ask you to complete the New Patient Information packet that is included with this welcome kit. Please make sure you:

Complete the New Patient Registration Form.

Complete the Health History Questionnaire.

Review and sign the Notice of Privacy Practices.

Complete the Authorization of Release to furnish information and Assignment of benefits (Private Insurance, Medicare, and Medicaid).

Review the Contract Health Booklet.
